At Evergreen Valley Home Health, we provide high-quality care at home for adults, seniors and/or people who have occasional or recurring needs, such as
Provide services including disease management, post-surgical care, and wound care. Our experienced case managers will coordinate and guide each patient’s individual plan of care.
Our licensed physical therapists assist in recovery from fall injuries, joint replacement surgeries, and stroke-related disabilities. Helping patients restore function through exercises, gait training, use of assistive devices, balance training, fall risk prevention program.
Speech therapists help patients with cognitive training to restore language and communication skills and improve quality of life.
Help the patient re-learn to perform daily tasks such as bathing, grooming, and dressing to restore their independence and mobility. We also conduct a fall-risk assessment at the patient’s home and recommend safety improvements.
Consult with the patient regarding financial, social, and/or transportation challenges and help establish long-term plans, and offer crisis intervention and counseling.
Under the direction of the licensed nurses and clinicians, Home Health Aides assists with bathing, dressing, meal preparation, and household tasks as needed.
